Our Legal Duty
Coriolis Digital is required by law to maintain the privacy of your protected health information (PHI). We are also required to provide you with this notice about our privacy practices, our legal duties, and your rights concerning your PHI. We must follow the privacy practices described in this notice while it is in effect.
How We May Use and Disclose Your Information

Substance Use Disorder (SUD) Records: If we receive records protected under 42 CFR Part 2, we will only disclose them with your written consent or as permitted by law. These records will not be used in civil, criminal, or legislative proceedings against you without a court order or your specific consent.
Your Rights Regarding Your PHI
Access: You have the right to look at or get copies of your PHI.
Amendment: You have the right to request that we amend your PHI if you believe it is incorrect.
Accounting of Disclosures: You have the right to receive a list of instances in which we disclosed your PHI for purposes other than treatment, payment, or healthcare operations.
Restrictions: You have the right to request that we place additional restrictions on our use or disclosure of your PHI.
Breach Notification: You have the right to be notified following a breach of unsecured PHI.
Redisclosure Notice
Information disclosed pursuant to this notice may be subject to redisclosure by the recipient and may no longer be protected by the HIPAA Privacy Rule.
